Michigan Surgery Specialists, P.C. is actively involved in our communities. We are pleased to announce our newest service to our surrounding schools, which is providing sports physicals to our local athletes. This is a new service for our practice which we are eager to launch. We are assisting Regina High School’s Athletic Department by providing physicals for the students at a nominal fee, with all proceeds being donated to their Athletic Department. Click on the resource tab to find your Michigan High School Athletic Association Medical History Form , Physical Exam & Clearance & Consent Form. These forms can be printed out for you to complete.
